Practice Patient Services
PROCEDURES INCLUDING SKIN CANCER SURGERY, HORMONE IMPLANTS, AND MIRENA INSERTION
We offer a wide range of procedures including skin cancer excisions, skin curettage and diathermy, flap and skin grafts for skin cancer wound closure if required, hormone implants and Mirena insertion. The practice has a three bay treatment room with a ceiling surgical light.
We consider immunisations to be an important part of primary health care, have a vaccine fridge, and provide childhood vaccinations. All immunisations are entered into the Australian Immunisation Register. We give Influenza vaccines to all ages through influenza vaccine clinics which usually run in April and May each year. Travel and work related vaccines can be given, and we are a registered World Health Organisation Yellow Fever vaccine centre.
Practice Nurses provide triage, advice, assessment, monitoring, education, tests, and procedures.
Are you 75 years old or older? If so, you might benefit from a new service being offered by Dalyellup Family Medical Centre. We are now offering comprehensive health assessments for patients who are 75 years of age or older. The health assessment consists of a visit to your home by one of our Nurses, Lucy. She will collect relevant information which assist your Doctor with your ongoing management. This will usually take less than an hour. The Nurse will also ask about your lifestyle, nutrition and social supports and provide you with information that may be helpful to support you. A follow up consultation at our Practice with your usual Doctor will then be arranged to complete the health assessment, during this visit an ECG (test to check your heart) may be done. The Doctor will do a physical examination, review all your tablets, and arrange further examination or investigation if required. The health assessment is funded by the Commonwealth Government through Medicare and will be bulk billed to Medicare by our Practice. A health assessment is only available to people 75 years or older who live independently. It is recommended by the Doctors at our practice as it is a valuable opportunity to provide a comprehensive health assessment for you, on a yearly basis.
PRE-EMPLOYMENT MEDICALS
We can provide pre-employment medicals, and secure urine drug and alcohol collections occur within the building.

We offer asthma education clinics for people living with Asthma. The appointment includes an education session with Lucy our Registered Nurse .
The asthma educator session will include:

An assessment of severity of your asthma

Spirometry testing (lung function) if appropriate,

Asthma self-management including consideration of asthma action plan and immunisation.

Understanding Asthma – signs, symptoms and triggers

How to use medication and spacers correctly

You will need to see your GP first to make an asthma plan and please bring this with you as well as your medication and spacer to the session with Lucy.

Standard consultation fees apply to the GP appointment and Spirometry test, while there is no cost for the asthma educator session. Children under 12, Health Care Card holders and Pensioners will be bulkbilled.

CHRONIC DISEASE MANAGEMENT INCLUDING CARE PLANS
Dalyellup Family Medical Centre can create, with your assistance, a care plan to improve the management of your condition or conditions that will enable us to coordinate care for you with other healthcare professionals. Some examples of chronic diseases are diabetes, asthma, ischemic heart disease, fatty liver, and osteoarthritis. VIDEO CONFERENCING WITH SPECIALISTS
Videoconferencing enables you to have a video consultation with a remote specialist (in Perth or the Eastern States). A GP or Nurse from the Practice is present during the consultation. To read more about how videoconferencing operates click here video-consultation-information-for-patients
